What is collected and why?

The form collects your name, age, general location, email address, face photograph, answers and confirmations. This information is used only to consider your application, decide whether John would like to contact you, communicate with you and help keep the process safe.

Your application is likely to reveal information about your sexual orientation. This is special category personal data. It is processed only with your explicit consent, given using the checkbox before submission.

Legal basis

The legal basis for processing your application is your consent. You can withdraw that consent at any time by emailing the address above. Withdrawing consent does not affect processing that took place before withdrawal.

Who receives the information?

Your application is reviewed by John and is not published. FormSubmit processes the submission so it can be delivered by email, and relevant email and website-hosting providers may process limited information to provide their services. Information will not be sold or used for advertising.

FormSubmit may retain written submissions for up to 30 days; its submission archive does not retain uploaded files. You can read FormSubmit’s privacy information.

How long is it kept?

If John does not pursue an application, the application and photograph will normally be deleted within 90 days. If you begin communicating, relevant information may be kept while you remain in contact and for up to six months after the last contact, unless there is a safety or legal reason to retain it longer. You can request earlier deletion at any time.

Your rights

You may ask for access to your information, correction of inaccurate information, deletion, restriction of processing or a copy of information you supplied. You may also withdraw consent. Requests should be sent to the email address above. You can complain to the UK Information Commissioner’s Office if you are unhappy with how your information is handled.
